The Philippine Statistics Authority (PSA) has officially opened Philippine Identification System (PhilSys) registration for children 1 to 4 years old. The PSA now allows PhilSys National ID for Minors , children can obtain National IDs. The PSA is continually making efforts to make the registration process child-friendly; our registration personnel are ready to assist our young registrants and their parents in the process.

Criteria

Children are eligible to receive either the physical national ID cards (PhillD) or  digital ID (ePhillD) of the ID. Philippine Identification System (PhilSys) allows parents and guardians to register their children aged zero to four years old . Their demographic data and full biometric information will still be captured once they reach the age of 15.

First of all access to  website portal and make sure online appointment;

Visit PSA Registration Centre

Go to the nearest designated registration centre along with PhilSys ID and request for PhilSys National ID for Minors.

Accomplished Form 

Complete the application form under the Child National ID criteria with accurate information.

 Present Documents for Minors ID

One should present following documents for acquiring PhilSys National ID for Minors;

1. Certificate of Live Birth issued by the PSA or the Local Civil Registry Office
2.  Report of Birth issued by the PSA or Philippine Foreign Service Post (PFSP
3. Certificate of Foundling issued by the PSA
4. Certificate of Foundling or Certificate of Live Birth or Persons with No Known Parent/s issued by the PSA
5. Municipal Form No. 102
6. Any document showing the full name of the child, date and place of birth, the full name of the mother, and the full name of the father, if acknowledged.

          After verification of  all given documents and information, PSA registered the minor from age 0 to 4 and allot              PhilSys National ID for Minors Registration Number.
